Healthy Families Parenting Inventory (HFPI)
User Agreement & Terms of Use

(by purchasing HFPI training registration and/or manual and Tip Sheets you agree to these terms of service)

1. After purchase of the HFPI User Manual, LeCroy & Milligan Associates, Inc. grants permission to use the Healthy Families Parenting Inventory (HFPI) free of charge provided the user agrees to be bound by all of the stipulations set forth by this Agreement and submits the information required on this electronic registration form.

2. the user acknowledges that the Intellectual Property for the conceptual models, instrument and User Manual listed in this Agreement belongs to LeCroy & Milligan Associates, Inc.;

3. the user shall properly cite the authorship of the instrument (Citation: LeCroy & Milligan Associates, Inc., 2004) and HFPI User Manual (see copyright), including but not limited to citing the authors on the survey forms that are handed out to participants and/or used for online data collection, on all training materials, and citing the authors in any subsequent reports and/or publications based on the data collected with the instrument;

4. the user shall not modify the content of the HFPI, the composition of the scales, the order and the wording of the items, the response framework and response labels, the scoring rubric, or the title of the HFPI and the titles of scales in the HFPI; however, the user may modify the visual formatting of the instrument;

5. the user may use separate scales provided all other conditions in this Agreement are met;

6. the user shall provide to LeCroy & Milligan Associates, Inc. copies of reports and/or publications based on the data collected with the HFPI;

7. the user shall not sell or grant permission to use the HFPI to third parties;

8. It is further agreed that LeCroy & Milligan Associates, Inc. will be informed ahead of time about all presentations, posters, speeches, etc. that involve the HFPI and appropriate authorship credit is given in each instance;

9. Indemnification: the user expressly agrees to hold harmless LeCroy & Milligan Associates, Inc., and the authors of the instrument from all liability, including but not limited to all expenses, costs, harm and damages of whatever kind, which might arise out of any use of the HFPI included in this Agreement.

In addition, the user is encouraged to contact LeCroy & Milligan Associates, Inc. to discuss potential grant opportunities, evaluation services, training services and various research projects that might involve the HFPI.
